Data Sharing

There are 3 forms of data sharing that you need to be aware of, and make decisions on:

1. Summary Care Records.  Core information on medication and allergies are summarised, and made available in the event of an emergency to A&E / hospital.  By default you will have a summary care record, unless you wish to opt out.  For further information visit the Connecting for Health Website. If you choose to opt out of the scheme, then you will need to complete a form and bring it along to the surgery

 

2. Data sharing in and out.  This is about how all your medical information is potentially accessed by different medical professionals, in different medical organisations, to provide you with the best level of care.  By default you are opted out of data sharing, and so need to make a decision to opt in (which is something that we support).  However, all children aged under 12 will by default be opted IN to data sharing.

With your permission, we can allow this system to “share out” your GP records with other healthcare providers (e.g. hospitals and clinics) should you ever visit them.  These other providers will also ask your permission to view your records prior to them then accessing it.

We will also need your permission to “share in” information that has been added to your records by other healthcare providers so that we can see that information.

Sharing your records will help the NHS to deliver the best level of care for you, because wherever you go, the medical staff will be able to see your medical history.

3. care.data   NHS England have commissioned a modern data service on behalf of the entire health and social care system.  This involves the anonymised extraction of key clinical information to inform national trends and decision making around the commissioning of new services.  By default you are opted in.  If you would like to access further information please go to www.nhs.uk/caredata or telephone 0300 4563531.   If you would like to opt out, please advise the reception team when you are next at the practice, or phone the practice to let us know.

We hope you will agree to share in and share out. We think it will make life safer for you, minimise delays — for example, in finding out the results of tests — and make it easier for us to deliver high - quality care to you safely and effectively. (And whatever choices you make, you can change them whenev er you want. You can always say ‘No’ — at any time you wish.)
